Saint Barnabas Health Care System's Home Health Care
Services offer a wide variety of health care services
in the comfortable environment of your own home.
We offer a full spectrum of home health care services
which includes skilled nursing care, enterostomal
therapy, specialized wound care, infusion therapy,
venipuncture and laboratory services, maternal child
health services, pediatric care, rehabilitation services,
personal care services, pain management, dietary
consultations and medical social work (for counseling
and connections to community resources). The focus
of care is to provide you with the home health care
and support you need to achieve quality of life,
maximizing your independence to the highest possible
level.

Kare are accredited by the Joint Commission on the
Accreditation of Healthcare Organizations and Medical
Center Health Care Services is accredited by the

Who conducts the initial assessment
to determine the services needed?

The Registered Nurse or the Physical Therapist will
go through a thorough assessment to identify the
patients' needs. They will then determine the most
appropriate level of care to allow you to achieve
quality of life, maximizing your independence to
the highest possible level. The teams of professionals
work closely with your physician to coordinate care
in your home or place of residence.

What geographic areas
do the agencies serve?

Our agencies serve the following counties in the
state of New Jersey:
- JerseyCare Home Health covers Essex County
only.
- Medical Center Health Care Services covers
Essex and Union County and portions of Morris
County.
- Community Medical Center Home Health covers
Ocean County only.
- Community Kare covers Ocean County and Southern
Monmouth County.

Is the service available
24 hours a day, 7 days a week, including holidays?

After business hours, our agencies have an on-call
service. Phone numbers are provided once service
begins.

What insurance plans do you take?

Medicare, Medicaid, private and managed care insurance,
no fault and workers compensation, as well as self
pay.

Do you provide services in Assisted
Living Facilities?

Yes, an assisted living facility is the patients'
primary residence. In addition to services covered
by Medicare and Medicaid, private duty care is available
by calling Medical Center Health Care Services for
Essex County, Union County and portions of Morris
County: 973-243-1001 or by calling Community Kare
for Ocean County and Southern Monmouth County: 732-905-1776.

Medical Center Health Care Services participates
in the Personal Care Assistance (PCA) Program offered
by Medicaid.
