Home Care offers a wide variety of career opportunities for the health
care professional to utilize his or her skills. For individuals who
like working one-on-one with patients and their family members, a career
as a Home Care Nurse, Physical Therapist, Occupational Therapist, Speech
Therapist, Nutritionist, or Medical Social Worker may well be a perfect
fit. In the patients' home environment, the health care professional

Positions in the fast-paced hospital environment
are also available for experienced home care nurses, as Home Care Intake
Coordinators. In this position, the nurse helps plan the patients' discharge
from the hospital, and assists the patient and family to make the transition
back home. The Home Care Intake Coordinator explains the services available
to the patient, and works with the physician, the case manager, as well
as with the agency to develop a plan of care for the patient after discharge.
